How To Make A Claim

Please submit the relevant Claim Form from the links below.

The Critical Illness and Health Cash Plan claim forms should be completed and submitted online with the required documentation attached. You may also download these forms and send them via email or post.

The Life Assurance and Personal Accident claim forms should be downloaded, completed, signed and sent to Reed Rewards (with a copy of the required documentation) via e-mail or post.

Critical Illness

You are required to submit your claim form within 180 days from the date on which you are made aware of your condition. Pre-existing health condition/s are not covered by the policy.

Health Cash Plan

You are required to submit your claim form and all relevant receipts within 90 days from the date on which you received the treatment/services. Pre-existing health condition/s are not covered by the policy.

Life Assurance

Notification should take place as soon as reasonably possible. Pre-existing health condition/s are not covered by the policy.

Personal Accident

You are required to submit your claim form and all relevant particulars within 90 days from the date of occurrence. Pre-existing health condition/s are not covered by the policy.

Professional Indemnity

Notification should be made on your first awareness of either a shortcoming in your work for a client which is likely to lead to a claim against you, or of any claim or threatened claim against you.
